According to Gav Thorpe and his game, Inquisitor, the Inquisition was created when four scholars speculated on the internment of the Emperor within the Golden Throne. Two believed that the Emperor was to be left alone and that his final wishes were to continue on with his body being a beacon that would guide humanity against the Warp while his soul followed whatever path the Emperor was destined for. The other two believed that the Imperium could not survive without the Emperor and sought to resurrect him, pulling his soul back from the warp and restoring his body.
Since such a thing may either save mankind or doom it, and the Emperor made no mention of this possibility before his internment, the first two sought the new Senatorum Imperialis and the remaining Primarchs for aid against the resurrectionists. Thus, the Inquisition was started as an organization to prevent the doom of Mankind|Imperium. Originally, they recruited other people who were smart enough to understand the nature of the Warp and the potential harm.
However, the Inquisition in its final form as guardians of the Emperor and of all Mankind, though started then, did not become its full form until the 32nd Millenium|M32. There were originally only two Orders within the Inquisition. They were created to work against the forces of the Ordo Xenos|Aliens and the Ordo MaelleusDemonic. Later, a third was added after the events of the Age of Apostasy proved that there are heretics within the Imperium that also proved themselves as a threat to Mankind.
