The Imperial Ravagers' Fortress Monastery orbits the Imperium world Vindict V, though it is not known if that is the Chapter's Homeworld. In M39, they received an Astropath sent order from the Ordo Malleus Inquisitor Jaq Draco, to perform an Exterminatus on the Hive World Stalinvast; due to it being infested with the Hydra warp entity. What the Chapter did not know however, was that Draco had changed his mind and withdrew the order for Exterminatus, after the Hydra entity disappeared into several Warp portals. Unfortunately, the Astropath Draco had used, Moma Parsheen, had sent the order without the Inquisitor's knowledge. She was the personal Astropath of Stalinvast's Planetary Governor and bore a grudge against the world and its people; for being locked in a sanctum beneath the Governor's palace for most of her life. This was her chance for revenge and by the time Draco learned of this, he was unable to contact the Imperial Ravagers to withdraw the order1a and Stalinvast was destroyed.1b
