The chief passion passions of Painboyz are surgery and extreme dentistry, the latter is often carried out during the former, as [[Teef]] are a commonly accepted form of payment for their services. Painboyz learn their craft through a mixture of genetic memory, trial and error, and the Ork concept of "Orky know-wotz". Their greatest joy are is conducting 'eksperimentz', and in many ways this makes Painboys the biological equivalent of the [[Mekboy]]. And just like Mekboyz, Painboyz are often accompanied by [[GrotOrderly|Grot orderlies]] orderlies who aid them in their surgeries as well as other menial tasks.{{Fn|18}}

Painboyz are responsible for fixing injuries that the Ork physiology can't deal with, such as totally severed limbs and brain damage. Seen as a As Doks are often considered not quite right in the head by the rest of their kind, an Ork will only go to a Painboy when he has no other choice, as these Oddboyz are infamous for trying out experimental procedures (such as the greatly feared [[squig]] brain transplant) on patients while they are under anesthesia (known as "concussion" to other races). Painboyz are also responsible for attaching [[bioniks]], although sometimes they aren't paying attention and replace the wrong part of the patient's body. However , Orks are surprisingly resilient and have had arms, legs and even heads swapped around and the Ork in question has survived to tell the tale!{{Fn|4}}

Painboyz are still Orks, and during battle will take part in the violent revelry with the rest of his ladz. They will cheerfully take part in combat, wielding [[Power Klaw]]s, [[Dok's Tools|surgical saws]] and other medical instruments to dismember foes. They will also wield a variety of [['Urty Syringe|deadly fungal serum syringes]], some of which can cause the hapless victims ' blood to congeal with spores. In battle, they are frequently accompanied by the same [[Grot]] orderlies that aid them during their medical procedures.{{Fn|18}}

The Painboy desire to conduct experiments does not extend simply to fellow Orkoids however. During battle they will seek out foes to capture and study, conducting all sorts of horrifying procedures and vivisections without anesthetic on screaming victims. Other victims will be transformed into horrifying cybernetic monstrosities. Some will be ripped to pieces for no particularly particular reason other than the Painboy wishing to tinker around in something. Painboyz are naturally inquisitive in their craft, and upon seeing any new kind of foe will immediately rush to capture them for experimentation.{{Fn|9}}{{Fn|18}} Some particularly ambitious Painboss's have even attempted to capture [[Space Marine]]s so they may unlock the secrets of their [[Gene-Seed]] and produce a strain of even hardier Orks.{{Fn|19}}

There exists a subclass of Painboyz known as "Bad Doks". All others pale in their eccentricity compared to these Painboyz and they are outcasts of their own tribes due to the unnecessary surgery they inflict upon their prey. Patients who fall victim to a Bad Dok can find themselves with mechanical lungs, extra limbs, or their head transplanted onto a new body. For a Bad Dok there is no need for payment, the surgery itself is a reward.{{Fn|1b}}{{Fn|4}}

High-ranking Painboyz are called '''Painbosses''' and are known to be accompanied by [[Cybork]] bodyguards.{{Fn|1a}} They specialize specialise in both surgical care and cyber-augmentation of both Ork and [[Squig]]. So as to provide their services "on the hoof", Painbosses augment themselves with piston-driven soopa-legz, monowheels, and other bizarre forms of [[Bionik]] augmentation.{{Fn|8}} Capable of leading an entire [[Waaagh!]], they will raid entire whole [[system]]s in search of new "interestin' specimens" upon whom to unleash horrific experiments upon.{{Fn|9}}
