Uphrateus Incident
The Uphrateus Incident is the Imperium's ongoing campaign to reclaim the world Uphrateus.[1]
History
It lies close to the Eye of Terror and was once the Homeworld of the Sentinels of Uphrateus Chapter, as well as a vital base for guarding against Chaos incursions. However this ended in M41, when Uphrateus was engulfed by the Eye and the Sentinels were thought lost to the Imperium. Because of this, their name was stricken from the Roll of Chapters afterwards and with Uphrateus being lost within the Eye, no effort was made to reclaim the Sentinels' Homeworld. It was coded for eventual erasure from the Imperial Records, but while the influence of Chaos had turned Uphrateus into a world of madness, the Sentinels survived and remained loyal to the Imperium.[1]
Uphrateus soon became infested by the forces of Chaos, due to the Eye of Terror, and the Sentinels began a campaign to reclaim their Homeworld from the invaders. This brought them into conflict with the Word Bearers of the Chaos Lord Saegius Mindblighter, who had constructed a necropolis upon Uphrateus. Mindblighter preached his heretical sermons from a cathedral there and this was later attacked by the Sentinels. Despite suffering grievous casualties, the Chapter was able to successfully breach the cathedral's defenses and in doing so, they also weakened the grip the Eye of Terror had on their Homeworld. This allowed Uphrateus to break free of the Eye and after 200 years it returned to Realspace.[1]
This was soon discovered by the Imperium and a major operation was launched to reclaim Uphrateus. Once this was done, the world would once more be used as a base of operations against the forces of Chaos. When the Imperials arrived at Uphrateus, though, they learned that not only was Uphrateus heavily corrupted, but that it also contained numerous heretics, traitors and Daemons. The Sentinels' survival and continued loyalty was discovered as well and the Imperials learned the Chapter was fighting within Mindblighter's cathedral. However operatives of the Imperium's Scholastica Psykana became alarmed about the battle, as Mindblighter was manipulating the bloodshed in order to summon a vast Daemon host of Khorne to his aid. Seeing a full picture of Uphrateus' fate led the Imperium to send two of the Space Wolves Great Companies to aid their forces. However they would not arrive in time stop the Chaos Lord's summoning from being completed. In desperation, the Officio Assassinorum was ordered to send the Eversor Assassin Tormentius to Uphrateus, in order to stop that from happening. Once he arrives there, the Imperium has estimated that Tormentius will have three hours to infiltrate Mindblighter's cathedral and kill the Chaos Lord, before the summoning can be completed.[1]

The Sentinels of Uphrateus, their Homeworld, Tormentius and Saegius Mindblighter, were created by the Games Workshop employees Wayne England and Ian Pickstock for Inferno! 1.[1]
